Liability Waiver & Photo Release.

Assumption of Risk & Release of Liability

I understand that participation in athletic training involves inherent risks of physical injury. In consideration for being permitted to participate in training with A.I.E. Sports Training, I hereby release, waive, and discharge A.I.E. Sports Training, Coach G, and its representatives from any and all liability arising from participation, to the fullest extent permitted by law. I certify that the athlete is physically able to participate and I authorize emergency medical treatment if necessary. This waiver is binding on my heirs, executors, and assigns.

Photo & Video Release

I grant A.I.E. Sports Training permission to photograph and record video of the athlete during training sessions, events, and related activities. I authorize A.I.E. Sports Training to use, reproduce, publish, and distribute such photos and video — including on the website, social media, marketing materials, and promotional content — without compensation. I waive any right to inspect or approve the finished materials and release A.I.E. Sports Training from any claims arising from their use. If the athlete is a minor, I confirm I am the parent or legal guardian with authority to grant this release.

Agreement

I have read and agree to the A.I.E. Sports Training Liability Waiver and Photo & Video Release. If the athlete is a minor, I confirm I am the parent or legal guardian with authority to sign on their behalf.
